Sometimes the referer is listed as "bookmark". I don't know how that
can happen. It is rare, so not too important.

In any case, I think what I will do is load a couple of files into a
test directory and experiment a bit to see how my accesses get listed
on my log stats.

If "no referer" does indeed mean that the visitor typed the URL
directly into the address field in their browser, then my site has a
very high ratio of site visits to home page accesses! That wouldn't
surprise me and it is not real critical that they all find their way
to the home page so long as they can find it if they want.
