survey script

I'm looking for a cgi script to offer a survey on my web site.
I'm on a win2003 server and don't believe I'm set up to run ssi.

I'd like to have different surveys on different pages.........
probably run them in a table nestled in a column on the page.
Ideally when someone "votes", the contents of the table changes
to show the results on all the voting.

Any idea of something available? preferably free :-) ?
